How to File a Complaint

Complaints & Decisions

Before you make a complaint to CNNB, you may wish to discuss your concerns directly with the nurse’s employer.

CNNB cannot accept complaints by telephone and cannot accept anonymous complaints. Read the Complaints CNNB Can Address and Complaints Process sections of our website which includes helpful information on completing the form.

There are two versions of the Make a Complaint form: an online form and a downloadable MS Word form. You will need to include details about your complaint, including the date, the nurse and the facility. Having this information on hand will make it easier to file your complaint.

Upon receipt of your complaint, CNNB will send an acknowledgment letter to you.

To make a complaint using the MS Word form, please follow the steps below:

  1. Download and complete the File a Complaint form
  2. Once you have completed the form, save it as a PDF document and email it to complaints-plaintes@cnnb-opinb.ca or print it and mail it to:

The nurse(s) involved will be provided with a copy of your complaint and will be informed of who made the complaint. The nurse(s) will be given the opportunity to respond to the complaint, and a copy of any response received (except for personal health information, if applicable, as discussed under Third Party Complaints) will be sent to you.
